Transport Canada, Marine Safety has the mandate to assess the fitness of seafarers in Canada. Transport Canada (TC), Marine Safety agrees to accept the appropriately completed medical assessments (form HC/SC 3312E or F) completed by Health Canada (HC) Occupational Health Medical Officers (OHMO's)/designated physicians for CCG seagoing employees in accordance with the Marine Personnel Regulations and the Canada Shipping Act (2001). The Health Canada medical assessment (tear-off) will be accepted as a "Provisional Marine Medical Certificate".
TC also agrees to accept the HC medical assessments for non-certificated CCG seagoing employees who are assigned to vessels that have a TC voyage classification of International/Unlimited voyages. The HC medical assessment (tear-off) will be accepted as a "Provisional Marine Medical Certificate". CCG will provide to TC/Marine Medicine Branch a list of HC OHMO's who provide medical assessments for CCG seagoing personnel.
CCG will stamp the HC/SC 3312 indicating "Consent to Release Medical Information" for the signature of the employee, and have the consent witnessed and dated. Also indicated on the aforementioned stamp will be the Certificate Type and the Canadian Maritime Document Number (CON No.), and a checkbox for Non-Certificated/International voyages (see Annex A).
The HC OHMO office will submit to TC a copy of the assessment with a copy of the completed Occupational Health Assessment Report (noted above) to the Marine Medicine Branch of TC, in Ottawa. Copies of assessments will only be sent to TC if the CCG stamp is properly completed and signed by the employee assessed under seagoing occupations.
Should TC deem it necessary to discontinue acceptance of the HC medical assessments, TC will provide CCG with at least six months notice of such an intention.
The Director, Personnel Standards and Pilotage (TC), and the Director, Marine Personnel (CCG) will liaise regularly to maintain open communication with respect to the application of this MOU.
TC undertakes to disseminate information as to its commitments under this MOU to their regional Marine Safety branches, thus ensuring national consistency and proper application of the process.
CCG will provide HC with a copy of this agreement for clarity purposes.
