Potential incident analysis format example

The potential incident analysis (PIA) can be a stand-alone document or incorporated into your emergency response assistance plan (ERAP). There is no specific format for a PIA. However, you can use the following example of a completed scenario as a starting point.

Example of a complete scenario

Title: potential incident analysis (PIA) for Chem Industries Inc. ERAP 2-XXXX

Scenario 1

Anticipated release of dangerous goods: TC412 carrying hydrochloric acid involved in a rollover (e.g., slid down an embankment and is currently five feet below grade on its side) with no apparent loss of contents.

Consequences

  • Possible injury to driver and other persons
  • Possible damage to property
  • Vapours are toxic: inhaling of vapours could produce irritation to respiratory system
  • Product is corrosive: contact could result in burns to skin tissues and eyes
  • Means of containment may be precariously positioned
  • Integrity of the means of containment may be compromised
  • Possibility of a leak due to container not being upright
  • Valves and fittings may be inaccessible or may not function as designed
  • Means of containment may need to be repositioned to facilitate off-loading
  • Possibility of other fluids from the vehicle being released (e.g., fuel, brake fluid, coolant, etc.)
  • Road closures
  • Public concern about the dangerous good
  • First Responders may not have been trained to respond to a hydrochloric acid release

  • Ensure public safety by reducing or eliminating any hazards based on the training and the personal protection equipment (PPE) available to them
  • Phone the ERAP telephone number, if able, to provide an ERAP incident report to Chem Industries Inc.
  • Provide the shipping document to the authority having jurisdiction (AHJ)/Incident Commander (IC)
  • Provide information on the cause of the incident
  • Provide information on the means of containment to the AHJ/IC and/or ERAP team leaders
  • Aid in the damage assessment of the means of containment, if trained to do so

If they received the ERAP incident report in lieu of the ERAP technical advisor, then:

  • Confirm incident details with the carrier and fill out an incident assessment formFootnote *
  • File an ERAP implementation report with the Canadian Transport Emergency Centre (CANUTEC) when implementing the ERAP to either Tier 1 or Tier 2 and contact ERAP team leaders, ERAP response teams and/or mutual aid partner, if needed
  • Maintain contact with the incident site through the AHJ/IC, carrier, ERAP technical advisors or ERAP team leaders
  • Source alternate compatible and compliant means of containment if a transfer is needed and ensure the driver is trained to transport dangerous goods.
  • Source heavy equipment to upright the means of containment when safe to do so

Tier 1 and Tier 2

  • After receiving the ERAP incident report from the carrier
    • fill out the incident assessment form*
    • make an ERAP implementation report with CANUTEC when implementing the ERAP to either Tier 1 or Tier 2, and
    • contact ERAP team leaders, ERAP response teams and/or mutual aid partner, if needed
  • Maintain contact with the incident site through the AHJ/IC, carrier or ERAP team leaders
  • Provide the incident assessment form*, safety data sheet, Emergency Response Guidebook guide 157 and other relevant technical information for the dangerous goods to the AHJ/IC and/or ERAP team leaders
  • Provide signs and symptoms of exposure to the AHJ/IC and/or ERAP team leaders
  • Provide information on the means of containment to the AHJ/IC and/or ERAP team leaders
  • Provide neutralization advice to the AHJ/IC and/or ERAP team leaders
  • Provide decontamination advice to the AHJ/IC and/or ERAP team leaders

Tier 2 only

  • Mobilize and deploy to the incident site if requested by the ERAP team leaders or senior management of Chem Industries Inc.
  • Once present, provide advice on the dangerous good or means of containment, as needed
ERAP team leaders

Tier 1 and Tier 2

  • Provide emergency response advice to the AHJ/IC and/or ERAP technical advisor, as needed
  • Maintain contact with the incident site through the AHJ/IC, carrier or ERAP technical advisors

Tier 2 only

  • Mobilize and deploy to the incident site if requested by the ERAP technical advisor or senior management of Chem Industries Inc.
  • Ensure all necessary equipment is deployed to the incident site
  • Wear appropriate PPE as dictated by monitoring results and/or the site safety plan
  • Monitor for presence of dangerous good using pH paper and/or hydrochloric acid gas detection tubes
  • Perform a site assessment (may be done with appropriately qualified and trained responders)
  • Perform damage assessment (may be done with appropriately qualified and trained personnel)
  • Develop site safety plan and do the safety briefing for all those involved in each operational period
  • Develop the incident action plan for the upcoming operational period for the AHJ/IC
  • Ensure standard operating procedures are followed for the response actions performed by the ERAP response team
  • Ensure that decontamination is set up prior to entry to the hot zone.
  • Perform necessary response actions with ERAP response team, if needed
  • Ensure any transferred or repackaged dangerous goods are safe for transport and are compliant
  • Ensure any waste generated during the response is packaged properly and taken to an appropriate facility
  • Work with the carrier, AHJ/IC, and other trained persons (e.g. heavy tow truck operators) to ensure public safety during the reorientation of the means of containment
ERAP response team

Tier 2 only

  • Wear appropriate PPE as dictated by monitoring results and/or the site safety plan
  • Monitor for presence of dangerous good using pH paper and/or hydrochloric acid gas detection tubes
  • Confinement – defensive measure (i.e., damming, diking, etc.) to ensure that if a release happens, it or any runoff from vapour knockdown is confined to a defined and manageable area away from the public, waterways, sewers and other exposures
  • Containment – if the means of containment has been breached, tighten or repair valves. Plugging and/or patching may be done to reduce or eliminate the release
  • Transfer the product – truck to truck transfer done following the standard operating procedure (including grounding and bonding)
  • Neutralize the product or its residues, if necessary
  • Decontaminate personnel and equipment

Tier 2 only
