To apply for a Canadian endorsement, please download and follow the instructions contained within the following Transport Canada forms:

Supporting documents to submit

  • Application for a Canadian Endorsement form completed and signed by the foreign seafarer.
  • Declaration of the Authorized Representative form completed and signed by the authorized representative of a Canadian-flagged vessel who intends to employ the seafarer. This should include:
    • the Labour market impact assessment (LMIA) number; or
    • documents, such as a work permit, that prove the foreign seafarer meets the requirements to work in Canada.
  • Valid medical certificate issued either by Canada or by a state whim whom Canada has a reciprocal arrangement.
  • Evidence of sufficient knowledge of one of the official languages of Canada.
  • Evidence of sufficient knowledge of Canadian maritime laws.
  • Copy of the Certificate of Competency and, if applicable, the STCW endorsement (all pages) to be endorsed;
  • Copy of the Certificates of Proficiency to be endorsed, if required (all pages).
  • Proof that the seafarer has completed the required Marine Emergency Duties (MED) or refresher training courses.
  • Any pertinent training certificates that the seafarer holds.
  • Valid foreign passport.
  • Passport-sized photo, taken within the last six (6) months.
