I agree to permit a representative of Transport Canada to conduct audits of this facility at any time during working hours. I understand that the Certificate of Registration is issued on condition that conformance with requirements of the standard, as determined by an audit, is maintained.

I have attached an uncontrolled copy of the quality assurance program documentation (including procedures, forms and sample markings) for the preparation, leak testing, inspection, refurbishing, marking and record keeping operations relating to:

____ the leak-test and inspection of Intermediate Bulk Containers (IBC's), including:

          Metal IBCs___Plastic IBCs ___Composite IBCs ___and/or

___the inspection of Mobile IBC's (UN Standard Mobile IBC, UN 31, TC 57 and ULC 142.13).
